Travel Clinic hours are Monday-Friday, 9:00 a.m.-4:30 p.m.

Plan to visit us a minimum of two weeks prior to your trip to allow your body to respond to vaccines.

Prior to Your Appointment
Notify your primary care physician that you are planning to travel and inquire about any potential complications related to vaccines. Request a copy of your immunization record.

We will ask you to complete our Patient Health Questionnaire. You can download it here. Bring this form with you on the day of your appointment.

The Day of Your Appointment
Plan to arrive at least 20 minutes before your scheduled time. Bring your immunization record with you. If you have already completed the health questionnaire, bring that with you as well. Plan to spend at least 30 minutes with our travel nurse.

Fees and Payment

$25 one-time consultation fee (does not include immunizations)

Immunization fees vary.

Payment is due on day of service. We accept cash, checks, Visa, MasterCard or American Express.

Some insurance plans cover travel clinic services. You will receive an itemized bill to submit to your carrier.

If your insurance covers Travel Clinic services, please complete the Insurance Authorization Form and bring it with you on the day of your appointment.

If your insurance plan does not cover your Travel Clinic fees, please complete an Insurance Waiver Form and bring it with you on the day of your appointment.
