Post-acute Services

Post-acute Services is comprised of Skilled Nursing Units on the Davies and California campuses. Patients on these units have completed their acute hospitalization, but continue to need rehabilitation and/or nursing and medical services. Patients are encouraged to work toward their optimal level of function. Patient and family education is an integral part of rehabilitation providing patients the living skills to function as independently as their condition allows. Family participation is encouraged and family education occurs throughout rehabilitation.

Our Skilled Nursing units offer a warm, healing atmosphere emphasizing a personalized approach to recovery focusing on physical and psychological well being.

General Admission Criteria

- Established need for skilled nursing services
- Medically stable
- Completion of pre-admission assessment
- Evaluated and approved for admission by California Pacific Post-acute Intake staff
- Expected to tolerate and benefit from a skilled rehabilitation program
- Potential for community reentry
- Verification of healthcare benefits for skilled nursing services

We use an interdisciplinary team approach in all skilled nursing programs at California Pacific.

Team specialists include:
