    Referral Guidelines

    California Pacific's Regional Rehabilitation team is available to assist with your rehabilitation needs. Review the patient referral program and general acute rehabilitation criteria, or telephone us with a specific query.

    Patient Referral

    CPMC's Regional Rehabilitation Center is known as a center of excellence in acute physical rehabilitation, we accept referrals from physicians, case managers, discharge planners, commercial carriers and other healthcare organizations utilizing general acute rehabilitation criteria.

    Our acute rehabilitation services are specifically designed to meet the rehabilitation needs of individuals who suffer complex or catastrophic injuries or illnesses such as:

    • Brain Injury

    • Spinal Cord Injury

    • Stroke

    • Amputation (single or multiple)

    • Cardiovascular Disease

    • Congenital Deformity

    • General Debilitation

    • Hip Fracture

    • Multiple Trauma

    • Neurological Disorders
      • Multiple Sclerosis

      • Polyneuropathy

      • General Medicine

      • Transplant

      • Muscular Dystrophy

      • Parkinson's Disease

      • Guillain-Barre

      • Other Pogressive Neurological impairments
    • Polyarticular Arthritis
      • rheumatoidarthritis

      • psoriatic arthritis

      • seronegative arthropathies

      • severe or advances osteoarthritis
    • Oncology

    • Orthopaedic Injury

    Co-Morbidities May Include:

    • Ventilator/Tracheostomy

    • Dialysis

    • IV/pumps

    • NG or PEG tube

    • Heparin Drip

    • Wound VAC

    • Bariatric

    General Acute Rehabilitation Criteria

    • Demonstrates need for multidisciplinary rehabilitation program of at least two disciplines (includes rehabilitation nursing, physical, occupational and/or speech therapy, and caregiver training)

    • Able to participate in multi disciplinary therapy program

    • Requires 24-hr nursing care

    • Community discharge plan home (home, home with assistance, home with day-program, board and care or assisted living)

    • Adult – 18 years of age or older

    • Expected rehabilitation potential – functional improvement

    • Medically Stable

    • Appropriate rehabilitation benefits and financial responsibility has been determined
