Patient Referral
CPMC's Regional Rehabilitation Center is known as a center of excellence in acute physical rehabilitation, we accept referrals from physicians, case managers, discharge planners, commercial carriers and other healthcare organizations utilizing general acute rehabilitation criteria.
Our acute rehabilitation services are specifically designed to meet the rehabilitation needs of individuals who suffer complex or catastrophic injuries or illnesses such as:

General Acute Rehabilitation Criteria
- Demonstrates need for multidisciplinary rehabilitation program of at least two disciplines (includes rehabilitation nursing, physical, occupational and/or speech therapy, and caregiver training)
- Able to participate in multi disciplinary therapy program
- Requires 24-hr nursing care
- Community discharge plan home (home, home with assistance, home with day-program, board and care or assisted living)
- Adult – 18 years of age or older
- Expected rehabilitation potential – functional improvement
- Medically Stable
- Appropriate rehabilitation benefits and financial responsibility has been determined
