CPMC's Acute Rehabilitation Admission Information
Rehabilitation begins upon admission. Members of the rehabilitation team conduct a comprehensive individualized needs assessment.
The team sets realistic goals and develops a individualized treatment plan. Progress is monitored, goals are evaluated and counseling is available to address issues associated with disability.

Acute Rehabilitation Accommodations
All rooms are equipped with an adjustable bed, private bathroom, television and telephone. Our hospital is a smoke-free environment therefore, smoking is not permitted inside the facility.
All meals are freshly prepared and served in the patient dining room. Group dining is considered part of the rehabilitation program.

Personal Items to Bring
When away from home, personal items always help make a patient's stay more comfortable. We encourage patients to wear their own clothing during their stay with us. Use the suggestion lists below to help plan your stay with us. All items should be tagged with patient's name.
Suggested Clothing List:
Items need be loose fitting, wash-and-wear apparel that is easy to put on and remove.
- 3 to 5 Front button or pullover shirts or blouses
- 2 Pairs of loose fitting slacks/shorts
- 2 Pairs of sweatshirts & sweatpants
- 3 Bras
- 3 to 5 T-shirts
- 1 Set of pajamas and robe
- 1 Pair of slippers with non-skid soles
- 1 Weeks supply of undergarments and socks
- 1 Pair of comfortable walking shoes
- 1 to 2 Sweaters or a light jacket
- Favorite bed pillow
- Personal medical equipment such as braces, splints, crutches, canes and wheelchairs
- Preferred grooming or toiletries (tootbrush, razors, deodorant, cosmetics, shampoo, etc.)
Please - NO perfume or cologne - Hairbrush and comb
- Glasses
- Hearing aid
- Dentures
- Non-electric razor
- Family photo
- Medications list with dosage instructions
- Copy of patient's Living Will or Durable Power of Attorney
- Patient's admission paperwork provided by the admitting physician
- Insurance identification cards

Visiting Hours
The support of family and friends is a very important part of rehabilitation. We encourage visitors Monday through Friday between the hours of 3:00 pm and 8:00 pm; and Saturday and Sunday from noon until 8:00 pm.
Your therapy requires a good deal of energy and time, so we ask that your guests be considerate of your scheduled therapy sessions and your rest periods.
The following are general guidelines for visitors:
- Please remember, if you have a cold, sore throat, or are generally not feeling will, you should not visit patients.
- To protect our patient's privacy and to ensure patient confidentiality, visitors may be asked to leave the room during tests or treatments or when the doctor or nurse needs to see the patient.

Financial Arrangements
Medical care at California Pacific Medical Center is covered by most insurance plans. Insurance companies are contacted for prior authorization before admission. Most insurance plans will follow patient progress throughout their hospitalization.
Government (Medicare, Medi-cal, Medi-caid) and private insurance providers have clinical and financial criteria that must be satisfied. This criteria is reviewed with you upon or prior to admission. The clinical requirements for a rehabilitation hospitalization will vary depending on the level of care required.
