To pre-register for Physical Therapy, Occupational Therapy, Speech Therapy, and Hand Therapy Services, follow the steps below.
Step 1: Completion of the Out Patient Services Pre-Registration Form

Step 2: Appointment Scheduling
We will contact you after your pre-registration is received to schedule an appointment based on a time and day preference you provide.
