Applying for a Class Scholarship
The Institute for Health & Healing is committed to assisting class students who cannot afford the full class fees. We award financial aid through class scholarships that are made available to individuals based on financial need. Scholarships are made possible through generous contributions from grateful patients and donors. We are especially thankful for Mrs. Roberta Neustadter's long-standing support.
To request a scholarship, please fill out an application. If you are requesting more than $50 in scholarship or financial aid, to complete your application, a copy of your most recent tax return is required. We require this information and your signature on the scholarship application:
- Name
- Address
- Phone (daytime and mobile)
- Have you ever received financial assistance or a scholarship from the Institute for Health & Healing clinic or community class programs?
- Name of the class you'd like to take
- Amount of scholarship request
- Annual household income
- Number of individuals in your household (specify adults/children)
- Health condition or other reason for seeking to take the class
