Multidisciplinary Case Management
At California Pacific Medical Center’s Heart Failure and Transplantation Program we are committed to a comprehensive team approach in treating your patient. Whether a patient is awaiting a transplant, having a VAD implanted or in recovery from surgery, sensitive and difficult challenges await—for them and their family. We provide support with our experienced, highly-trained cardiac care team, comprised of:

- cardiovascular and thoracic surgeons

- a nutritionist
- licensed clinical social worker
- psychologist
- financial counselors
- rehabilitation specialists
Patients also benefit from education and support available through the
Community Health Resource Center, the
Women’s Health Resource Center, the
Institute for Health & Healing, and the
Center for Patient and Community Education. We believe this multidisciplinary patient care approach leads to high patient satisfaction—and to more successful outcomes.
