Many studies have clearly shown that good blood sugar (glucose) control can prevent or delay the complications of diabetes. The healthy lifestyle that can control diabetes also helps to prevent other physical problems. Education is recognized as key to managing this common but serious condition.
The Center for Diabetes Services' mission is to provide comprehensive education that gives persons with diabetes the information, tools, and strategies to self-manage their disease, and prevent or delay the onset of complications.
The Center for Diabetes Services uses a team approach to address all aspects of diabetes treatment and management. Our nurses, dietitians and social worker coordinate care with your physician to assure that you receive education on diet and exercise, monitoring blood glucose, and medications. Personal, family and financial issues that affect self-care are also considered.
You are encouraged to attend diabetes management classes. In addition, you will have a private meeting with the dietitian to individualize a meal plan, and a meeting with the nurse to review your care plan and progress as needed. If you need help adhering to the plan, you are encouraged to meet with the social worker to help with stress management as well as goal setting.
Individual sessions can be arranged for patients who do not speak English, have impaired hearing, or upon request as needed. Clinical staff members include Cantonese, Spanish and Tagalog speakers. Interpreters are available for all languages, including American Sign Language.
