The social workers at the Community Health Resource Center offer an array of direct and indirect services.

Laura Kleinman, MSW, Erin Tarica, MSW, and Scott Plymale, LCSW have extensive experience in the field of case management and psychotherapeutic counseling.

The services offered to the community are affordable and informative. CHRC offers support groups including; Caregivers' Café, Cancer Support Group, Repetitive Strain Injury Support Group, Hepatitis C Support Group, and a Trictelimania Support Group. In addition to the support groups, individual counseling is provided to community members at very affordable rates.

CHRC social work services are provided to a number of departments within the California Pacific Medical Center including; the Cystic Fibrosis Center and the Cardiac Rehabilitation Center. Contact CHRC to make an appointment to meet with a social worker.

Counseling Services

  • Assessment, crisis intervention, short-term counseling and case management addressing the complex psychological impact of illness, aging and other lifestyle concerns
  • Collaboration with medical professionals in planning and obtaining services for patients, families and loved ones
  • Information on and referrals to a wide variety of community resources and governmental agencies and programs, including patient advocacy and assistance
  • Stress management counseling
  • Referrals to therapists for ongoing treatment


  • Finding the Right Resources

    After the initial shock of being diagnosed with a condition or an illness wears off, people are often bombarded with resources to turn to for assistance. Social workers at CHRC can help find the right resources for each individual, whether his or her needs can be met right at CHRC, within the community or a governmental agency. Social workers can also refer clients to patient advocacy groups if necessary.
