Coming Home Hospice
Coming Home Hospice is a residential hospice program providing care and support for clients and their families facing terminal illness. When care at home is no longer an option, Coming Home Hospice can provide a safe, loving and warm place to reside during the final period of life. Caregivers provide 24-hour nursing and personal care in a home-like atmosphere giving families the freedom from worry about care and safety issues so they can focus on their loved one.

When do you Choose Coming Home Hospice?
Coming Home Hospice is chosen as a care alternative for patients in the final stages of life who can no longer be cared for or live in their own homes. We are here when end-of-life issues seem insurmountable.

Volunteering at Coming Home Hospice
Volunteers provide assistance and companionship to clients at Coming Home Hospice. California Pacific Medical Center's Department of Volunteer Services is part of the California Pacific Medical Center Foundation, which is dedicated to developing resources that support the Medical Center in providing access to the best health care available. More information about volunteering with Coming Home Hospice is available here or call us at 415-600-0374.
Coming Home Hospice has been made possible by the generous support of donors to California Pacific Medical Center Foundation. For more information on the Foundation or on ways you may give, please call
415-600-4403.
Make a Gift
About one-half of the Coming Home Hospice budget is paid for by reimbursements from health insurance plans. The balance - about $750,000/year - comes to us in support from individuals making a gift to the Foundation at the California Pacific Medical Center.
We need and seek your support. Please make as generous a gift as possible. It is only because of your help that we are able to keep our doors open.
You can send your check to: CPMC Foundation/Coming Home Hospice, P.O. Box 7999, San Francisco, CA 94120.
Leave a Legacy: You can leave a lasting legacy to your community by naming the Coming Home Hospice as the beneficiary of your will, trust, retirement plan, or life insurance policy. Contact Leonard Graff at 415-600-2410 for more information.
Memorial Gifts: Your gift can be in honor of, or in tribute to, a loved one.
