Cancer Risk Checklist for Colorectal and Endometrial Cancer. If you can check off any of these boxes for either your mother's or father's side, you should further investigate your family history with the Cancer Risk-assessment Chart.

Colorectal and Endometrial Cancer Risk Checklist

O Personal or family history of colorectal cancer or endometrial cancer diagnosed before age 50.

O Personal or family history of multiple Hereditary Nonpolyposis Colorectal Cancer (HNPCC) related cancers in the same person (such as two colorectal cancers or both colorectal and endometrial cancer).

O Two first-degree relatives (parents, siblings or children) with colon or endometrial cancer diagnosed at any age.

Print out the Cancer Risk-assessment worksheet and place the appropriate number of check marks where you or your family members have had cancer as indicated.

If you have placed check marks next to two or more of the categories you may want to speak to your healthcare professional about assessing your risk of inherited Colorectal or Endometrial cancer.
