Cancer Risk Checklist for Breast and Ovarian Cancer. If you can check off any of these boxes for either your mother's or father's side, you should further investigate your family history with the Cancer Risk-assessment Chart.

Breast and Ovarian Cancer Risk Checklist

O Personal or family history of breast cancer before age 50

O Personal or family history of ovarian cancer at any age

O Personal or family history of breast and ovarian cancer in the same individual

O Family history of male breast cancer

Print out the Cancer Risk-assessment worksheet and place the appropriate number of check marks where you or your family members have had cancer as indicated.

If you have placed check marks next to two or more of the categories, or have had any male relatives with breast cancer at any age, you may want to speak to your healthcare professional about assessing your risk of inherited breast and ovarian cancer.
