To Make a Gift as a CPMC Employee

• Click here to access the current CPMC Employee Payroll Deduction Form. If you choose to give a recurring gift, the amount you designate will be deducted from each paycheck on an ongoing basis. Please complete the form and return it to Debra Capitan (see below).
