Bayview Child Health Center
Goal: $500,000
This neighborhood suffers from escalating rates of pediatric obesity and diabetes, the city’s highest rate of childhood asthma, and an infant mortality rate among the worst in California. Your support will help provide affordable primary, acute and preventive care services for thousands of medically underserved children in the Bayview-Hunters Point community.
Coming Home Hospice
Goal: $400,000
Coming Home Hospice is known for providing a peaceful, homelike environment at the end of life. Your gift will ensure that this compassionate, dignified care for terminally ill patients will continue to be available to those in need.
HealthFirst
Goal: $150,000
Based at St. Luke’s Health Care Center, this innovative program offers patients and other clinics access to prevention, outreach, and education services. Your gift helps HealthFirst integrate community health workers into a multidisciplinary medical team model and is measurably improving the management of chronic diseases like diabetes, asthma and others.
Institute for Health & Healing
Goal: $450,000
True healing mends not just the body, but also the mind and spirit. Funding ensures that the institute can continue its mission of serving more than 70,000 patients annually through its comprehensive, holistic in- hospital and outpatient programs. (Gifts are eligible for Sutter Health Matching Grant program.)
Kalmanovitz Child Development Center
Goal: $350,000
Your gift enables the KCDC to continue providing diagnosis, evaluation, treatment, and counseling for children with learning disabilities,developmental disorders, Down syndrome, cerebral palsy, and autism spectrum disorders.
Women’s Health Resource Center
Goal: $100,000
Your support enables this center to continue to provide more than 10,000 women each year with a place to obtain health information, resources for well-being through classes and support groups, and compassionate care for women with cancer.
