• Patient name, address, telephone
• Condition
• Authorization
• Primary payor
• Referring physician
• Full or shared-risk patient
• Clinic notes

Once the physician visit or hospital admission has been scheduled, our team will complete the process of coordinating the referral for you. Your physician will communicate directly with you, keeping you apprised of the patient's condition, treatment and outcome.
