    For Physicians: Kidney Transplant Referral

    California Pacific's transplant physicians are dedicated to transplant on a full-time basis and visit outreach locations throughout Northern California and Nevada to provide pre-transplant evaluation and post-transplant follow up.

    Referring a Patient

    Through an easy referral process, our program handles complex cases with referring physicians who co-manage patients' care. We also provide referring physicians with transplant and nephrology updates through local lectures and symposia.

    To refer patients for a pre-transplant evaluation at California Pacific Medical Center, contact us at our San Francisco location (415-600-1000) or through our outreach nurse coordinators located in the East Bay, Eureka, Fresno, Modesto, Napa, Redding, Roseville/Sacramento, San Francisco, the South Bay and San Luis Obispo.

    We are happy to review a patient's medical history to help decide whether he or she is healthy enough to make a good transplant candidate. In general, any adult with end-stage renal disease can be referred to California Pacific for a transplant evaluation. We encourage patients to be referred for transplantation before they begin dialysis treatments.

    Although there is no true age limit for transplantation, transplant recipients over the age of 60 cannot have any significant medical disease other than their kidney disease. Patients in older age groups may require more pre-transplant medical screening than younger patients.

    Once a patient has been referred (depending on insurance type, the referral can be made by the patient or a health care provider, though a referral letter with background medical information from a physician is optimal), our financial specialists will screen a patient's insurance to make sure that the patient is covered for the evaluation.

    After insurance clearance or "authorization" has occurred, the patient is scheduled for a transplant evaluation clinic visit. The scheduled visit can take place at the Medical Center in San Francisco or at any of our outreach sites. Throughout this process, we work closely with referring physicians and patients to ensure excellent ongoing communication and patient education.
