    Scheduling a
    Kidney Transplant Evaluation
    at California Pacific Medical Center

    If you or a family member are interested in having a kidney transplant evaluation at California Pacific Medical Center in San Francisco or at one of our outreach clinics in the East Bay, South Bay, Fresno, Modesto, Roseville, Eureka, San Luis Obispo, Redding or Reno, our transplant team can help with the process.

    Who Can Get a Kidney Transplant Evaluation?

    In general, any adult with end-stage renal disease can get a kidney transplant evaluation at California Pacific Medical Center. We encourage patients to be referred for transplant before they begin dialysis treatments. Although there is no formal age limit for transplant, patients in older age groups may require more pre-transplant medical screening than younger patients.

    To pursue evaluation, your physician can speak with our kidney transplant doctors about referral or you can complete the form on this page. Alternately, you can contact one of our outreach coordinators at the location nearest you.

    Steps Prior to Kidney Transplant Evaluation

    Before your evaluation appointment, our financial coordinator will screen your insurance and obtain the necessary authorization to make sure you are covered for the evaluation. If you belong to an HMO, this authorization is mandatory. For Medicare and Medi-Cal patients, eligibility and benefits are verified and authorizations obtained if necessary.

    After insurance clearance or "authorization" has occurred, the patient is scheduled for a transplant evaluation. You can read more about this process in our handbook. The scheduled visit can take place at the Medical Center in San Francisco or at any of our outreach sites. Throughout this process, we work closely with referring physicians and patients to ensure excellent ongoing communication and patient education.
