Referral Process
To refer patients to California Pacific Medical Center's Heart Failure and Transplant Program, physicians should contact us at (415) 600-1050. Depending on the nature of your question, Medical Director Ernest Haeusslein, MD, or one of our transplant nurse coordinators will speak with you regarding the patient's needs and schedule a heart evaluation if appropriate.
Because every patient with end-stage cardiac disease is not a candidate for transplantation, we work closely referring physicians and patients to determine if transplant is appropriate or if other innovative therapies should be pursued.
